Intensive Mental Health Services

Heritage Clinic provides intensive mental health services with older adults in parts of Los Angeles County and all of San Diego County. In partnership with the Los Angeles and San Diego County Departments of Mental Health, these extended services are delivered through a Full-Service Partnership for Older Adults Program. The Clinic extends its field-based mental health services to seniors by providing “whatever it takes” to support older adults with the most serious mental health difficulties.

  • Outreach and engagement
  • Intake
  • Assessment, including neuropsychological evaluation
  • Care management
  • Individual rehabilitation
  • Medication evaluation and follow-up
  • Crisis management
  • Peer advocacy and support
  • Client and caregiver support groups
  • Psychotherapy


Who may participate?

Elders whose optimal independence has been jeopardized by at least one of the following:

  • Homelessness and imminent risk of homelessness
  • Hospitalization and risk of acute psychiatric hospitalizations
  • Incarceration and risk of going to jail
  • Imminent risk of placement at a Skilled Nursing Facility or nursing home
  • Presence of co-occurring disorder
  • Recurrent, serious risk of abuse or self-neglect
  • Serious risk of suicide
